Monthly Order Form - PDF Download

A $15.00 monthly fee applies for postal deliveries. If ordering a 2 month supply, the postage will be $25.

Order forms for the next month’s supplies must be lodged with the Association by the 15th of the prior month. Goods will then be available for either pick up at our Arundel premises from the 1st of the month depending on the date your order is placed, or dispatched by postal delivery ( if so stipulated on the order form with the prepaid $15.00 parcel post fee submitted or $25 for 2 month orders ) on, or around the 10th of the supply month.

Instructions:

  1. Click the link below to open the document in a new window.
  2. Make sure you select Adobe Acrobat to open it so you can fill it in
  3. Fill in the form fields
  4. In the top left menu of Acrobat, click File > Save As >
  5. Save the file in a location you can remember (desktop or a folder of your choice)
  6. Send an email to gcoa@bigpond.com and attach your recently saved order form.

  2. Save the file in a location you can remember (desktop or a folder of your choice)
  3. Open the file once saved – fill in the form fields
  4. In the top left menu of Acrobat, click File > Save As >
  5. Save it in the same location

STOMA APPLIANCE SCHEME APPLICATION FORM

This form must be completed and lodged with the Association along with the covering joining fee before any appliance product order becomes effective.

Once your application form is processed, you will receive a government issued ‘Appliance Entitlement Card’ with your entitlement number. All orders must quote this number.

APPLICATION FOR ADDITIONAL STOMA SUPPLIES

This form when completed by a Stomaltherapist or Medical practitioner, makes provision under the Stoma Appliance Scheme for extra monthly ostomy supplies when the Ostomate has an extenuating stoma condition which has been properly assessed and diagnosed and justifies supplies in excess of the allowed monthly maximum allowances.

The member must sign this form on page 3 and tick the ‘Yes’ box.
